• Download and review the SEL Data Mining Opt-Out PDF
• Sign the Parental Opt-Out form on the last page of the PDF
• Return the signed Parental Opt-Out form to your cohort leader or directly to EPIC @ info@epic205.org (hard copy or email attachment)
• IMPORTANT STEP: Confirm participation by completing School Survey Opt-Out Online Form below

An EPIC representative will collect your signed opt-out forms and deliver them en masse to District 205 at the end of September. We are hoping to get 800-1000 signed Opt-Out forms at a minimum.

This is an easy way to make a big difference. If the District can’t get the data, they can’t justify the SEL/CRT agendas.
